    If you are filing a claim after the demise of the life insured, you will need to furnish the following documents.

    Accidental Death

    Accidental Death

    If the life insured has passed away due to an accident, your claim for Accidental Death cover will require you to submit the following documents. 

    Critical Illness

    In case of diagnosis of a critical illness, you will need to provide the following documents to claim a critical illness cover.

    Waiver of Premium

    Waiver of Premium

    If you are making a claim for Waiver of Premium, you will need to supply the following documents.

    To initiate your claim for Life Cover, you need to submit the following documents. However, if additional documents are needed, you will be required to furnish them as well.

    • Copy of Death Certificate (issued by local authority)
    • Original Policy Document (PD) or Indemnity Bond, in case original PD (physical copy) is lost. PD is not required, in case electronic PD is maintained in e-Insurance Account
    • Claimant’s Statement
    • Doctor’s certificate (in specified format)
    • Photo ID Proof and Address Proof of the nominee as per AML Requirements
    • Cancelled cheque leaf or bank pass book of the beneficiary under the policies

    To initiate your claim for Accidental Death, you need to submit the following documents.

    • Death Certificate (original or notarised copy)
    • Original Policy Document (PD) or Indemnity Bond, in case original PD (physical copy) is lost. PD is not required, in case electronic PD is maintained in e-Insurance Account
    • Claimant’s Statement
    • Doctor’s Certificate
    • Photo ID proof and address proof of the nominee (Refer Annexure for the list of accepted documents)
    • Attested copy of FIR/Panchnama/Iquest report
    • Attested copy of the Post Mortem Report/ Viscera Report
    • Cancelled cheque leaf or Bank Pass Book of the beneficiary under the policies
    • Copy of Newspaper Cutting (if any)

    To initiate your claim for Critical Illness, you need to submit the following documents. Please Note that Critical Illness Rider can be claimed only once the Survival Period is completed.

    • Claimant’s Statement
    • Doctor’s Certificate (in specified format)
    • Discharge summary from the hospital or attested copies of other hospital records (Admission Sheet, Bed Tickets etc.)
    • Original Policy Document (PD) or Indemnity Bond, in case original PD (physical copy) is lost. PD is not required, in case electronic PD is maintained in e-Insurance Account
    • Cancelled cheque leaf or Bank Pass Book of the life assured

    To initiate your claim for Waiver of Premium, you need to submit the following documents. 

    • Claimant’s Statement (in specified format)
    • Doctor’s Certificate (in specified format)
    • Original Policy Document (PD) or Indemnity Bond, in case original PD (physical copy) is lost. PD is not required, in case electronic PD is maintained in e-Insurance Account 
    • Original/attested copy of all treatment and diagnosis reports
    • Attested copies of other hospital records (Admission Sheet, Discharge Summary, Bed Head Ticket etc.)
    • Disability Certificate issued by the Civil Surgeon/competent specialist (06 months after the date of injury)
    • Certificate of current status of health
    • First Information Report
    • Cancelled cheque leaf or Bank Pass Book of the beneficiary under the policies
